Service for Blackburn's vulnerable adults

A service is available for vulnerable adults in Blackburn and Darwen.
It provides planned, short term, intensive assistance.
It is designed to maximise a person's independence after illness, disability or loss of confidence.
The service's aims are to:
- Enable service users to remain in their own home by promoting independence and prevent daily living skills being lost
- Prevent hospital admission n Support hospital discharge
- Improve quality of life
- Prevent dependence on domiciliary care
- Support family or carers
- Avoid admission to residential care.
Individuals are assisted, in their own home to boost confidence.
