When grumbling about life, the late, lugubrious and much-loved Les Dawson used to say: ‘I was a failure in the boom!’

Perhaps he should have worked for the NHS. Failure to run efficient hospitals or stop a proposed IT system wasting £12.7billion (how many A&E wards could have been kept open or cancer treatments given for that?) seems to result in promotions, telephone number salaries and pensions, even knighthoods.

As usual, we will be told that lessons will be learned and robust systems put in place. Sadly, history will repeat itself.
