A NEW scheme which should enable elderly patients to live independently at home has been launched at Blackpool Victoria Hospital.

The Aftercare Scheme for the Elderly is aimed at residents of Wyre or the Fylde over the age of 75 who may need support.

The team, co-ordinated by Melanie Heney and senior support worker Pat Nutter, will be based at the hospital's accident and emergency department.

"It can be very worrying for a patient following injury or illness in A&E," said Melanie Heney. "And they often wonder how they are going to cope when they get home.

"The aim of our service is to provide short-term support to help them overcome any immediate problems and to act as a bridge -- to help access or gain other beneficial services." Working closely with A&E staff, they will identify those patients eligible for support and contact them about the services the scheme offers. Once the patient has been discharged from hospital they will be contacted again to see if they would like a visit and some help with light housework and meals as well as help with shopping and collecting pensions.

Sister Lyn Robinson, emergency nurse practitioner for A&E, said she was delighted the scheme was up and running, adding: "The scheme will help patients to live independently in their own home and avoid having to be readmitted."
