A number of healthcare professionals, including nurses, doctors and ambulance workers have been charged, suspended or struck-off for misconduct while practising in Lancashire. 

The sanctions, imposed by the Nursing and Midwifery Council (NMC) this year, have included suspensions for mistakes made with medication, striking-off the nurses register for having indecent child images, and conditions of practise orders for botched botox injections.

Convicted Blackburn doctor to have case reviewed by tribunal

A doctor was to have his case reviewed after his fitness to practise was found impaired following a drug driving conviction.

The case of Dr Faisal Nawaz was heard in May, with the Medical Practitioners Tribunal Service confirming they will be reviewing the case.

Dr Faisal Nawaz, who qualified in 2017 and was in year two of his foundation training with the East Lancashire Hospitals NHS Trust, was convicted in 2019 for one count of dangerous driving and two counts of driving under the influence of cocaine.

Blackpool GP struck off for 'sexual assault' on patient

A GP was struck off after a tribunal ruled he sexually assaulted a patient and pressured her into a relationship by prescribing her excessive amounts of medication.

Dr Goskel Celikko, 76, groped his vulnerable patient at an appointment in November 1999, before taking her out for lunch, kissing her and ensuing a sexual relationship.

He used his authority to supply the patient, who was 22 at the time and had a history of mental health issues, with excessive amounts of medication during their relationship.

Dishonest Royal Blackburn Hospital nurse struck-off

A nurse who lied on patient’s records and who was "repeatedly dishonest" during her time working at an East Lancashire hospital was struck-off from the profession.

Joanna Louise Gaskarth-Parkhouse became the subject of an investigation after her colleagues at the Royal Blackburn Hospital noticed a series of issues with her work during a night shift on June 24, 2019. 

An investigation into her conduct found on more than one occasion between June 24 and 25, Mrs Gaskarth-Parkhouse claimed she had administered drugs such as antibiotics to patients on her ward, when in reality she hadn’t - and then subsequently falsified documents to make it look like she had done.

Blackburn care home nurse suspended for 12 months over medication mishaps

A former care home nurse was suspended for 12 months after a hearing found she had made a series of mistakes in administering medication.

Parveen Fatima, was employed as an adult nurse at Northwood Nursing and Residential Care Home in Blackburn, where it was alleged she failed to demonstrate knowledge, skill and judgment in medication administration, having administered medication to residents, and failing to sign the residents’ MAR charts.

Blackburn nurse suspended over medicine mishaps

An agency nurse working at Royal Blackburn Hospital was suspended after taking up employment as a registered nurse while she was subject to a conditions of practice order.

Ghazala Adman's fitness to practise was found to be impaired and she was given a substantive conditions of practice order in November 2019.

This was over alleged medication mistakes, and failing to maintain the dignity of a patient while he was using the toilet in incidents dating back to 2017.

She was told, among other conditions, she must inform the Nursing and Midwifery Council when taking up any other post, and these conditions were extended twice, most recently until May 2022.

Lancashire nurse risks striking-off order after being suspended from duties for medication failures

A Lancashire nurse risked a striking-off order if she did not engage with the Nursing and Midwifery Council, after being suspended for 12 months for being "incompetent".

Donna Antoinette Pickford was suspended from duties following a review of a fitness to practise order which was originally imposed on her two years ago.

Pickford, who was a registered nurse working in Blackpool, had a substantive conditions of practice order imposed on her for a period of two years by a Fitness to Practise Committee panel on May 21, 2020.

Lancashire mental health nurse struck-off register for lying after death of service user

A registered mental health nurse who was suspended from his role for six months for being 'dishonest' following the death of a service user has been struck-off by the Nursing and Midwifery Council.

In a second review of the suspension order originally imposed on Matthew Alexander Shaw-Smith, from Lancashire, who was a registered mental health nurse working at Cromwell House in Manchester, a panel decided to replace the suspension order with a striking-off order.

His original suspension order was for a period of six months and was handed down by a Fitness to Practise Committee panel on May 5 last year, but was extended for a further six months at a review hearing in December.

Five health care professionals charged following major investigation into ill-treatment of patients at Lancashire hospital

Five healthcare professionals were charged with a number of offences, including unlawful sedation, as part of a major investigation into the ill treatment of patients at a Lancashire hospital.

The five people, who worked at Blackpool Victoria Hospital, were charged in May following a police investigation into alleged offences committed on the stroke unit.

In November 2018 police were contacted by Blackpool Teaching Hospitals NHS Foundation Trust after concerns were raised about the care provided to a number of patients.

Accrington care home nurse removes herself from register after giving resident wrong medication

A care home nurse who was facing charges over poor medication management, after giving the wrong dose of insulin to a patient, voluntarily removed herself from the Nursing and Midwifery Council register.

Judith Brierley joined the NMC register in November 1990, and on June 17, 2019, she started working at Addison Court Care Home in Accrington as a registered nurse.

At the end of September 2019, the home's Quality Improvement Lead carried out a routine review of a resident's care records and discovered on a number of occasions Ms Brierley had signed to say that she’d witnessed the resident self-administer insulin into their arm.

Lancashire nurse squeezed patient's infected abscess after botched botox injection

A nurse who administered Botox to a patient without taking relevant safety precautions, leaving her with an infection and abscesses on her face, has been made subject to a conditions of practice order.

Joanne Lisa Kilpatrick, a community practitioner nurse who is also trained in aesthetic procedures, was found to have her fitness to practice impaired at a Nursing and Midwifery Council misconduct hearing earlier this month.

Kilpatrick was initially charged with 18 breaches of the nurse’s code - which included attempting to squeeze an abscess on a patient’s face after it became infected - and 10 of those charges were proven.

Lancashire nurse struck off following indecent child images conviction

A  specialist practitioner working for North West Ambulance Service Trust who was convicted of possessing hundreds of indecent images of children has been struck off the nurses register.

In June 2021, Neil Rankin, 39, was convicted of three counts of possessing indecent images of children and sentenced to 12 months in prison suspended for two years.

He was also ordered to complete 100 hours of community service, 50 days of rehabilitation work and subjected to an electronically monitored curfew for three months.

Blackburn nurse altered patient's records to cover own back

A Blackburn nurse had been handed a 12-month conditions of practice order after she altered a patient’s records to cover her own failings, just two days before the patient died.

Julie Isherwood was employed as a registered district nurse by the Lancashire Care Foundation Trust and became the dedicated nurse for a patient in December 2015.

At the time she was treating the patient at home for a wound on his hand, but he was later admitted to Royal Blackburn Hospital on February 29, 2016, with a pressure sore in his sacral area, something Ms Isherwood had been made aware of but hadn’t actioned.

Nurse gave patient four times required dose of morphine

A nurse who incorrectly administered morphine to two patients while working at a care home in Lancashire has been struck-off.

Lesley Rowland was suspended for 12 months from her position as a registered nurse in April 2021 after it came to light that she had given the wrong dose of morphine to a patient on September 3 2019.

When her case came before the Nursing and Midwifery Council (NMC) for review on April 25 this year, the suspension order was extended for a further six months.

However, following a more recent hearing on October 28, the NMC made the decision to strike Mrs Rowland off the nurses register, a sanction that came into effect on December 2.

A&E nurse struck off over sexual assault conviction

An A&E nurse has been struck off after being convicted of groping the breasts of his female co-workers.

Adam Soothill began working in the emergency department at Royal Preston Hospital in 2011, but in March 2019, allegations were made against him by several members of staff.

These included inappropriate sexual behaviour, which involved touching, the flicking or touching of breasts, grabbing breasts, seven touching of naked breasts, and placing his hands between legs.

The assaults took place between 2017 and 2019 and as a result of the allegations, Soothill was suspended from work pending a police investigation.
