WITNESSES have been asked to help police discover what caused a fatal head-on smash on the A59.
Sameer Vohra, 34, of Blackburn, died when his silver BMW car collided with a Daf HGV on the A59, Samlesbury, just before 1.30pm yesterday.
The female passenger of the BMW was airlifted to Royal Preston Hospital with serious but non life threatening injuries.
A Peugeot people carrier ran in to the back of the car and three people were treated for minor injuries.
In the wake of the tragedy, BAE Systems revealed there were plans to install a mini roundabout at the junction of the A59 and Myerscough Smithy Road - very close to where Mr Vohra died.
The roundabout, which may have prevented the accident, was agreed when BAE granted planning approval for its aerospace park last year.
Sergeant David Tonge, who is leading the investigation, said: "I would like to appeal to anybody who may have seen the collision, or any of the vehicles in the moments before, to contact police.
Any amount of information may help us to find out exactly what happened."
Malcolm Adams, of BAE Systems, said it was a dreadful accident and numerous workers had heard the bang made by the impact of the vehicles.
He said: "A mini-roundabout and cycle lane will be installed along that stretch of road.
"The new entrance we are building further up the road will offer safer entry and exit which is a major issue for us."
Chris Holtom, county councillor for the Samlesbury area, said the accident was confirmation of his worst fears regarding the stretch of road.
He added: "From the Trafalgar junction to the Mellor Brook roundabout this is one of the most dangerous roads in Lancashire.
"This death shows the real threat this road can have.
"There are major safety concerns on this stretch and I am working very closely with BAE to make sure any alterations are done properly.
"I don't think people realise how dangerous that road is.
"I use it regularly and often I seem to be the only one driving at a sensible speed."
A worker at the Boddington Arms pub, near to the scene, said she had struggled to get in to work following the smash.
She said: "I came down Cuerdale Lane and it was all blocked off.
"And when I got to Mellor Brook I had to tell them I worked down here, for them to let me through.

i know sameer personally me and him were like brothers we were always together especially the last 4 weeks as he was moving back to blackburn i have known him since we were kids he is already married and has no bit on the side. the girl in the passenger seat is from blackpool and was the wife of one my friends i told sameer to give her a lift to blackburn.SHE was not having a affair with sameer he was not like that i know all you loosers were jeolous of sameer as he was cruising about in the flash cars bmw or aston martin he never had a bit on the side he has 4 kids and a wife for god sake if they read these comments eg "DAVE" what would they think of their father he never had no affair and what right do you to have to know about his personal life. If i were you please keep your comments to your self he has gone from this world and won't back to read your comments he needs duas not these hateful messages please pray for my brother instead of writing these rumours of him having an affair. he was a good man and was lucky to pass away on a thursday today at his jannazah rain was falling when we went to bury him this is a sign of that allahs rehmat has gone with sameer he got the month of rabbi ul awal how many people get this month. we all have to go from the world one day so pray for him and someone will pray for you when you have gone so stop spreading these rumours you have no right to judge someone only god has like "never judge a book by its cover" only god is here not you Pray and remember life is tooo short. Be prepared for your own destiny and pray for the deceased. We are all going to depart one day. Live a life that people will say nice thinks when you depart, dont hurt anyone and their feelings and be a good character to mankind. Well, I reckon there's probably over 200 comments on this site with reference to this article, and not a single one of them can answer my very reasonable and respectful question about what Mr Vohra did for a living... He drove around in an Aston Martin? those cars are not cheap - I work extremely hard in a legal job, and yet I will never drive an Aston Martin. So Mr Vohra must have been a very esteemed professional, surely? So come on, people.. what was he? Doctor? Lecturer? Researcher? Surgeon? Lottery winner? Oscar-winning actor? I mean, Mr Vohra was obviously a professional of sort sort, I honestly thin that his tribute comments should reflect what this wonderful man did for the world. Why the secrecy?
