17-stone Blackburn grandma 'too fat for operation'

A grandmother who weighs 17 and a half stone has been told she cannot have surgery because she is too overweight.

Evelyn Lang, 43, said consultants at the Royal Blackburn Hospital refused to operate on her ovaries, kidneys and bowels because of her size.

At 5ft 4in, her body mass index is 40.9. Health bosses say anyone with a BMI over 30 is obese.

The former care assistant, who has had to give up work because of her ill-health, said her weight was difficult to control because of the steroids she takes to control severe asthma.

Dr Colin Waine, chairman of the National Obesity Forum, said: "I understand the surgeons' concerns over increased risk, but there are obstacles to this lady losing weight, and instead of discharging her, the hospital should have referred her to a specialist weight-loss clinic so that she can have the surgery as soon as possible.

"For people who are ill, losing weight often is nowhere near as simple as eating less. Leaving the situation as it is and simply taking more painkillers is not the answer - people need a lot of support."

Mrs Lang, of Fishmoor Drive, Blackburn, has nodules on her kidney, cysts on her ovaries and needs an exploratory operation to get to the bottom of a bowel problem.

She accepts that none of the conditions are life-threatening but says she is in almost constant pain.

The operation to deal with her conditions was set to take place in July last year, but was cancelled on the day because Mrs Lang was found to be anaemic.

When she returned to the hospital in February, expecting to be given a second theatre date, she claims a surgeon told her: "You're too fat and I'm not going to operate."

When she went back again last week, she claims nurses told her she was being discharged as a hospital patient, and would have to continue managing her severe pain with high doses of morphine.

Mother-of-four and grandmother-of-five Mrs Lang said she was given no dietary or exercise advice from the hospital - a claim denied by bosses - and now feared becoming addicted to the strong painkillers.

Mrs Lang said: "There was nothing said about my weight when they were about to do the operation in the summer, so why have they started saying this now?

"I don't want to keep taking morphine - I have been on it a long time and they keep having to increase the dose because it's stopping working.

"I'm willing to lose weight and I've made an appointment with my GP to talk about it but I can't do a lot of exercise when I'm in so much pain, so I'm in a vicious circle."

She added that she was much heavier - weighing almost 20 stones - two years ago, when she was given surgery for a hernia.

A spokesman for the hospital said a range of health problems had meant it would be dangerous to operate on Mrs Lang, with her weight being a major factor, and that advice had been sent to her GP on health improvements needed before the operation.

The hospital's director of operations Val Bertenshaw said: "Under no circumstances would a medical professional advise that a patient be sent home and discharged without correct treatment or information from the professional concerned as to the reasons why a operation can not be carried out.

"Patient treatment and safety is our priority.

"The correct preparation for an operation is just as important as the operation, and to ensure that no complications arise during surgery the operating team and surgeon must ensure that a patient is strong and able enough to endure the strain and stress of an operation, so that the best outcome can occur."

The Lancashire Telegraph's medical expert Dr Tom Smith said Mrs Lang needed to lose at least three stones to decrease the risks.

He said: "Anaesthetics dissolve in the fat around the brain, and will dissolve in excess body fat in obese people, so it takes a lot more anaesthetic to put an obese person to sleep.

"Then there are more problems after the operation, because it takes them longer to wake up.

"The anaesthetic in the fatty deposits can later travel to the brain, meaning a real risk of the person never waking up.

"On top of that, the excess tummy fat rests on the chest when the person is lying down for an operation, restricting their breathing."

Mrs Lang said she was willing to take the risks and have the operation.

She said: "At the moment I have no life anyway. I even have to get someone to help me clean my house and I can't go out and do the things I want to do."

Evelyn, I do sympathise with your situation. I am about as big as you after an accident, and i too have been on steroids, and im on a combination of 22 tablets and painkillers a day, i have arthritis and stomach ulcers and IBS now too, I was never big before though. I know that once the weight creeps on, bit by bit, you cant see an end to it, and you feel that it controls you, and you feel powerless to do anything. I started my first ever diet 3 weeks ago to see if i could lose weight (and im still on the tablets) and ive lost over a stone so far, so it can be done - it just depends on whether you want it badly enough, and whether you havwe the willpower to change your eating habits. Although i said much the same as you - I dont drink alcohol AT ALL (rather than your "dont drink canned or bottled beer") I gave up smoking a few years back too - but although i ate relitavely healthy, it was the WAY i cooked my food - and snacking that put even more weight on, in addition to my meds. Evelyn, all i say, and not in a condascending manner, is be truthful with yourself, take a real look at your routine/diet and see if there are any small changes that you could make to help yourself... good luck

The surgery shouldn't be refused if the patient wants it done and Florence is right they would do it if it was life threatening which seems daft when her life is threatened with all her ailments anyway.
At the end of the day you sign a consent form before going to theatre agreeing to whatever procedures are needed and that applies to all weights of people.

Florence Nightingale, Blackburn says...

Firstly this lady is not too fat for an operation, it's just that the risks associated with the treatment she requires are more than if she lives with her conditions. Of course a hospital would operate if she needed emergency surgery - because the risks would then be shifted. It aint rocket science!

Secondly, an exploratory procedure is not 'surgery' - a camera is usually inserted through the back passage and the bowel can be examined this way. Not a pleasant experience and some tolerance is required but no hospital would refuse to do this because of weight.

It seems the telegraph's 'experts' have focused on the anaesthetic risks for this lady's surgery. You don't need to be an expert to know that there are alternatives such as sedations and local anaesthesia where the patient is not put to sleep and there is much less risk.

Finally, someone suggested this lady puts her hand in her pocket and 'goes private'. Don't you know that the surgeon's who work in the private sector are the same ones who work for the NHS? And these surgeons carry out even less high risk surgery privately because there are no emergency services or high dependency wards in most private hospitals!

Obviously this lady needs to look at her lifestyle. Only she knows if changes need to be made here eg if she smokes there is only one thing to do - give them up! My mother has asthma and still manages to smoke 20 a day. when she decides to give up there is a dramatic improvement within 3 weeks and her inhalers are rarely required. It's amazing to see - of course staying off the ciggies is hard and her willpower always lapses due to her hectic social life.

I think the telegraph's reporting here is typical of the tabloids - not all the facts are available and they are quick to make royal blackburn's dedicated surgeons out to be uncaring and picky when it comes to patients' needs. There have been many many patients with much greater body masses operateed on - in fact Evelyn looks almost anorexic compared to some of them!

God you've not got a clue. You judge this woman because of where she lives. As someone mentioned STEROIDs alone put excess weight on people. Why don't you visit the many heart lung disorders of the hospitals & see how big some of those are.Whilst I was in for assessment there was a lady opposite, not a word of a lie she could not do anything for herself, she was the size of the hospital bed, BUT she'd had a heart transplant & was probably on high dosage of steroids.Of course the hosp. still treated her & I expect IF she had needed an operation she would have had that also.Go on prove it for yourslf, go on a steroid course of tablets, just for a few weeks.Lets see how bloated & big you all look then.I agree with steve from accrington.TOO many immigrants getting free NHS service & the british people are being put aside for whatever excuse or reason they choose to give.OPEN your eyes to the real world, in our hospitals etc.

People - the woman is on *steroids.* They cause weight gain; look it up. The woman has *asthma.* That is a lung disease (if you need it spelled out, look that up too.) She is on morphine because she is in constant pain. So these callous comments to "eat more" and "get off her fat arse" are completely misdirected.

It sounds to me like the NHS is just trying to weasel out of treating patients.

Because of this ladys size though and they wont operate on her at the moment makes me wonder though if she had an accident and needed operating on as an emergency would they operate?

My understanding is that the prime rule of the Hyppocratic Oath (apologies to any doctors if the spelling is incorrect!) is "do no harm". That means that, in their opinion now, it would be risky and harmful to operate. However, if she had an accident or a serious ailment, then it would be more harmful NOT to operate. So the answer, Lydia, is "Yes", they would operate in those circumstances. As for the lady herself, I don't know her or her circumstances and will not therefore pass any comment.

Robbie, B/Burn says...

While i agree some of the comments are harsh and hurtful, some are very true.

peoples joints and muscles hurt as they are not used to excercise and due to the stress a person weight is putting on them, once someone starts to get used to the exercise and lose weight the pain will start to stop.

you dont have to get out of bed to excerise. you dont have to go to the gym. or run. you dont have to do sit ups. or make much effort you can excercise from your sofa, and bed.

a good balanced diet, couple that with simple excercise while your sat down. (aka moving your arms. raising your legs.) can help you weight to lower.

i think the main problem (and i am not saying its the same with everyone) is people are lazy these days, they just can not be bothered to exercise.

steve, accrington says...

Lydia, I agree with you entirely. This is the start of a slippery slope.Smokers are threatened with not getting treatment as they say it`s self inflicted.Every time a smoker buys a twenty pack, at least £4.50 goes to the treasury, so smokers more than pay towards the N.H.S. than non-smokers. Now they are picking on obesity. It`s another excuse to cut costs as this government has allowed uncontrolled immigration from Europe, flooding our hospitals, schools, housing e.t.c. with more people, but not putting the finances in place to deal with all the extra costs. So everybody had better beware, because it won`t end here. What next? will it be sports injuries? if you play footie and you hurt yourself, tough ? I`m using footie as an example as a lot of injuries occur in this sport, but you can see how things could go.Cost cutting at any price, and any excuse to save money, rather than tackling the real problems facing this country.
