3. Request an application forms by emailing us here. We will then email an entry form back.
Downloaded applications should be returned by post to Business Awards Entries, Editorial, Lancashire Telegraph, Newspaper House, High Street, Blackburn, BB1 1HT.
Please enclose any supporting evidence along with your completed form.
