Hazel HOOK On Tuesday, February 17, 2009, peacefully in hospital, Fortified by the Rites of Holy Mother Church, Hazel, aged 83 years. The dearly loved wife of the late Jack, and a much loved mother, grandmother and great-grandmother. At rest in the Grimshaw Park Funeral Home. Requiem Mass at Sacred Heart Church, Preston New Road, Blackburn on Wednesday, February 25, at 2.00 p.m. prior to cremation at Pleasington.
